Website Change Update |
We had planned to move back to our old website address at www.haileyville.k12.ok.us. and to discontinue this website (haileyville.ok.schoolwebpages.com) on October 16. Due to unforseen circumstances, we have put these plans on hold.
We will be continuing with this website for the remainder of the 2009-2010 school year. Look forward to our website change late in the summer of 2010.

The E-Rate Family Income surveys must be completely filled out in order to be valid. Surveys with missing names, missing signatures, or missing information cannot be used. Submitting an incomplete survey or no survey only hurts our students (your children) by denying available funding to our school. Please be sure that your E-Rate Family Survey is filled out completely and is readable. |

Important - Submit Your Free And Reduced Price Meal Applications And Fill Out Forms Completely! |
When parents fill out the Free And Reduced Price Meal Applications, our school and our students benefit in many ways!
It is very important to fill out the forms completely every year. We try to obtain and maintain school funding for various programs and equipment which benefits all students. In order to obtain this funding, we must have high participation in the Free and Reduced Meal Program. Incomplete forms cannot be used and prevents us from receiving funds that could help our students.
Thanks to increased participation during the last few school years, we were able to get funding that was previously unavailable to us. We were able to purchase new equipment and technology to better enhance student learning and to increase communication with the public. Please fill out an application even if your child does not eat school meals.

Website Consent Forms |
Parents: Please read the following carefully. If you sign the Website Consent Form, your child will not be featured on our website or in any school submitted newspaper article for the 2009-10 school year.
Haileyville School recognizes our students for their achievements and school participation by posting articles and pictures on our website and in local newspapers. Some parents prefer that their children remain anonymous. To respect these wishes, we include Website Consent Forms annually in student enrollment packets. These forms are to be signed and sent back to the school
only if you do not want your child's picture posted on our website or submitted to newspapers by the school. These forms enable parents to opt out of website and newspaper recognition. The forms are available to download on the Document Manager page in the Forms category.
Our policy for posting pictures is as follows:
We do not post names with pictures. We only post names for articles, awards, or organizations if there is no picture available. In short, no names with pictures and no pictures with names unless we have parental permission.
We do post articles that have appeared previously in newspapers and magazines. Some of these contain pictures and names.
If the Website Consent form is not signed and returned to the school, permission is implied and the student's picture may be posted to our website and/or submitted to newspapers.
If you have signed the Website Consent form and your child appears in a group picture, the picture will be digitally altered to hide the child's identity.
Because of the large number of students and pictures involved on our website, mistakes are possible. We ask for your patience and understanding. Mistakes will be corrected as soon as they are found or brought to our attention.
Some parents think that it is okay to put student articles in local newspapers, but not on the school website. Most newspapers have websites and post articles online. Haileyville School will not post your child's picture to either the school website or newspapers if the form is signed and returned.
Website Consent forms are good for the current school year only. A new form must be signed and returned to the school each year to be valid. |

New School Reports Available To Download |
Oklahoma state law provides that "each school site shall notify the students' parents of the school's performance levels in the Oklahoma School Testing Program as reported in the Oklahoma Educational Indicators Program at the end of the school year." In accordance with this law, Haileyville Public Schools makes school reports available to the public through its website, local newspapers, and/or notes sent to parents.

There will be a new daily schedule for Haileyville High School this year. Students will have to start school a little earlier in the day, but they will also be dismissed earlier.

Important Information for Parents About Meningococcal Disease and Vaccines |
Senate Bill 1467, which became effective November 1, 2006, requires school districts to provide information about the health risk of meningococcal meningitis to parents and guardians of students. It also directs the State Department of Education and the State Department of Health to develop resources and information for local school boards on the prevention of meningococcal meningitis.
The immunization service of the Oklahoma State Department of Health prepared a meningococcal meningitis fact sheet for distribution to the parents or guardians of all students Grades 6 through 12. The fact sheet is available on our Document Manager page in the Information category. Go to Document Manager and click on the Information category to download the fact sheet InfoMeningococcalDisVacc[1]. The fact sheet is also available on the State Department of Education's website under "What's New."
